Use the official 2026 fillable application to nominate a qualified individual for the Order of Saint Martin. Before submitting the packet, confirm the appropriate award level, membership status, approving authority, and supporting-document requirements.

Prepare a Complete Nomination Packet
Review the award criteria before completing the application and gather the information needed to prevent unnecessary processing delays.
Select the Award Level
Confirm whether the nominee is being considered for the Distinguished or Honorable Order and meets the applicable rank and Quartermaster-service criteria.
Verify Membership
Provide the nominee’s AQM member number. If the nominee is not currently eligible, include the required membership application or enrollment.
Obtain Endorsement
Identify the O-6 or higher commander who is nominating or approving the individual and obtain the required signature or endorsement.
Attach Justification
Include a justification of at least one page describing the nominee’s service, accomplishments, and contributions to the Quartermaster Corps.

Complete each stage carefully and retain a copy of the finished packet for your records.

Assemble the Packet
Confirm the form is complete, obtain the approving signature, attach the one-page justification, and include membership documentation when required.
